A GP chronic condition management plan brings together your health goals, treatment and follow-up for eligible long-term conditions. Your doctor will assess whether a plan is appropriate for you.
The process may involve a nurse and GP appointment, with coordination of other services where clinically indicated. Contact reception to arrange a plan or a review of an existing plan.
